Special Accommodations Forms

Forms for requesting special accommodations are below. The following MUST be done or the forms will be returned to senders:

1. It is critical that all aspects of the GED testing accommodations forms be completed and that all processes are followed exactly as outlined in the forms and in the support material.  Incomplete forms and inappropriate content or diagnosticians will cause the form to be returned to the sender.

Also, from below, please study the “Guidance to Completing Forms” BEFORE completing any form to request accommodations.

2. Please be sure that a truly LICENSED DIAGNOSTICIAN conducts the assessment.

3. Please COMPLETE the correct form -- do not send attachments unless the correct form is completely filled out. Most forms also require a letter -- on the diagnostician's letterhead -- to accompany the request form.

4. Please be sure the diagnosis of the examinee is no older than the following:
. . . . . six months for Emotional and Mental Health (EMH) disabilities,
. . . . . three years for AD/HD conditions,
. . . . . five years for Learning and Other Cognitive disabilities (LD)
. . . . . current status for Physical/Chronic Health (PCH) disabilities.

Older documentation will be considered if that is all that the candidate can provide without undue burden or expense. Such special accommodation requests could take up to 8 weeks to process

Documentation and assessment tests must include a CLEAR diagnosis and provide information on current functional limitations that might affect the examinee's ability to take the tests under standard (normal) conditions, so that the rationale for the requested accommodations can be properly evaluated.

The licensed diagnostician's report must include certification or licensure -- and expiry date.

Typically, we prefer all requests for GED testing accommodations to be checked over and mailed via your preferred GED testing site. However, sometimes testing candidates wish to mail their requests directly to the Utah State Office of Education for processing.  The State mailing address is:
